 on the form Key areas include:

The Tax Registration Number (T.R.N) is especially important
as the application cannot be processed "Without it "
The regisnation number of the application. e.g. R.G.N or R.M. or B.A.N. 
is needed to verify the status of the applicant
Include a cell number for ease of communication
Last renewaI period to determine the period of licensure
Place of employment - To update our records
